Chronic Care Program

Our team of registered nurses can help you understand your chronic condition and how you can improve your health.

Our Chronic Care Program helps active SHBP/SEHBP members with chronic conditions better manage their health and improve their quality of life. These programs, which are offered at no cost to eligible members, reinforce your doctor’s treatment plan through educational mailings and support from our Care Managers, who are all registered nurses. The program is available for those members with:

  • Asthma
  • Chronic Kidney Disease (CKD)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Coronary Artery Disease (CAD)
  • Diabetes
  • Heart Failure

Our comprehensive Chronic Care Program offers eligible members:

  • A Care Manager with expertise in your condition
  • Access to a team of health professionals, including a registered dietician, social worker, behavioral health specialist and pharmacist
  • Assistance in navigating the health care system, understanding your benefits, and getting the appropriate care when you need it
  • Quarterly educational mailings

Who is eligible?

To find out if you're eligible for our Chronic Care Program, please call 1-888-345-1150.

To participate in our Chronic Care Program, members must also meet the following criteria:

  • Asthma Program: Members five years of age and older diagnosed with asthma
  • CKD Program: Members 18 years of age and older diagnosed with CKD
  • COPD Program: Members 30 years of age and older diagnosed with COPD (includes chronic bronchitis and emphysema)
  • CAD Program: Members 18 years of age and older who have been hospitalized for cardiovascular conditions like angina or a heart attack (acute myocardial infarction) or had procedures such as coronary artery bypass graft (also known as CABG) surgery, percutaneous transluminal coronary angioplasty or stent placement
  • Diabetes Program: Members of all ages diagnosed with diabetes
  • Heart Failure Program: Members 18 years of age and older diagnosed with heart failure

How to enroll

Eligible Horizon BCBSNJ members may enroll in the Chronic Care Program by completing the enrollment form or by calling 1-888-345-1150. Assistance is available Monday through Friday, from 8 a.m. to 7 p.m. Eastern Time

Are all programs free?
Yes, all programs are free to eligible members of Horizon Blue Cross Blue Shield of New Jersey. Participation in the programs will not affect your premium contribution or your status with the plan.

Which doctors can I see?
You should continue to choose doctors and other health care professionals as you usually do under your plan. If you need to find a doctor, specialist or care facility, use the Doctor & Hospital Finder.

Who can I contact with questions or to enroll?
Please call 1-888-345-1150 if you have questions or to enroll.

When is my case manager available?
Our case managers are available Monday through Friday, from 8 a.m. to 7 p.m. If you call outside of these hours, leave a voicemail message and your care manager will return your call the next business day.

How often will I receive information?
Once enrolled, you will receive an introductory letter and brochure informing you about the programs and how participation can help you better manage your conditions.

What if I join the program and then decide not to participate?
All programs are completely voluntary. You can call us at any time to cancel your participation.

Is my health information kept confidential?
Yes, we always work to ensure that all of your personal health information remains confidential.

Are durable medical equipment and prescriptions covered?
Participation in the program does not change your covered benefits. You are covered for durable medical equipment and prescriptions as outlined in your health benefits plan.

How was I identified?
You may have been identified either through claims data, pharmacy data, your doctor's referral or case management.
