Our team of registered nurses can help you understand your chronic condition and how you can improve your health.
Our Chronic Care Program helps active SHBP/SEHBP members with chronic conditions better manage their health and improve their quality of life. These programs, which are offered at no cost to eligible members, reinforce your doctor’s treatment plan through educational mailings and support from our Care Managers, who are all registered nurses. How to enroll
Eligible Horizon BCBSNJ members may enroll in the Chronic Care Program by completing the enrollment form or by calling 1-888-345-1150. Assistance is available Monday through Friday, from 8 a.m. to 7 p.m. Eastern Time
Are all programs free?
Yes, all programs are free to eligible members of Horizon Blue Cross Blue Shield of New Jersey. Participation in the programs will not affect your premium contribution or your status with the plan.
Which doctors can I see?
You should continue to choose doctors and other health care professionals as you usually do under your plan. If you need to find a doctor, specialist or care facility, use the Doctor & Hospital Finder.

When is my case manager available?
Our case managers are available Monday through Friday, from 8 a.m. to 7 p.m. If you call outside of these hours, leave a voicemail message and your care manager will return your call the next business day.
How often will I receive information?
Once enrolled, you will receive an introductory letter and brochure informing you about the programs and how participation can help you better manage your conditions.
What if I join the program and then decide not to participate?
All programs are completely voluntary. You can call us at any time to cancel your participation.
Is my health information kept confidential?
Yes, we always work to ensure that all of your personal health information remains confidential.
Are durable medical equipment and prescriptions covered?
Participation in the program does not change your covered benefits. You are covered for durable medical equipment and prescriptions as outlined in your health benefits plan.
How was I identified?
You may have been identified either through claims data, pharmacy data, your doctor's referral or case management.
